3. Do you or anyone in your family have medullary thyroid cancer?
4. Do you or anyone in your family have Multiple Endocrine Neoplasia type 2?
5. Have you ever had pancreatitis?
6. Have you had bariatric surgery in the past 12 months?
7. Do you have gallbladder disease, or was your gallbladder removed in the past two months?
8. Has a doctor told you your triglycerides are over 500?
9. Do you currently take insulin, a sulfonylurea, warfarin, or a GLP-1 prescribed for diabetes?
10. Have you used cocaine or methamphetamine?
